All affiliation forms for this season were posted to all member clubs in April. If you have not received your form or you are a new club wishing to affiliate, then a blank copy can be found on the right hand side of this page. You will need to print the Blank Cup form together with the Blank Affiliation form and submit both forms.
Please note if your club runs U18 teams, a Welfare Officer MUST be in place to affiliate.

Senior - £65.00
Intermediate/Women - £52.00
Junior – £42.00
Youth/Minor – £29.00
